Investigators say the pilots of a Brazilian passenger plane that crashed last month, killing all 62 people aboard, reported failure in the system to remove ice from the plane.

Investigators have released preliminary information on the still-mysterious crash of a Voepass airliner that killed all 62 on board in Brazil this summer.

Audio from the cockpit's voice recorder included comments from pilots indicating ice was accumulating and the deicing system was failing, Paulo Froes, an investigator in the air force's center for the investigation and prevention of air accidents, told reporters in Brasilia.

Officials say that while loss of speed alerts were triggered, the crew never declared an emergency before the plane crashed. The cause of the loss of control remains unclear, with Lieutenant Colonel Paulo Froes stating, “what we know is that the airplane was flying in an area with severe icing conditions”.
